Privacy Policy

How Forever Young LA collects, uses, and protects your information.

Effective Date: October 27, 2026

1. Who We Are

Forever Young LA is a medical aesthetics and neuropathy clinic located at 10138 Garvey Ave, Suite D, El Monte, CA 91733. This Privacy Policy explains what information we collect through our website (foreveryoungla.com), how we use it, who we share it with, and the choices you have. By using our website, you agree to the practices described here.

2. Information We Collect

We collect two categories of information: information you provide to us directly, and information collected automatically when you visit our website.

Information you provide:

  • Consultation requests — name, phone number, email address, service of interest, and preferred contact method when you submit our consultation form.
  • Patient intake — health information you enter into our secure, gate-protected intake questionnaire (used at our clinic via QR code). This is protected health information handled in accordance with HIPAA and our Notice of Privacy Practices.
  • Communications — the content of calls, emails, and text messages you send us, and your stated marketing preferences.

Information collected automatically:

  • Website analytics — when you browse our site, our analytics provider (Metricool) may record your IP address, browser and device type, pages visited, the website that referred you, and the date and time of your visit, through a small tracking pixel on our pages.
  • Technical data — standard web server logs, such as the pages requested and your IP address, which are used to operate and secure the website.

3. How We Use Your Information

  • To respond to your consultation requests and schedule appointments;
  • To provide and coordinate your medical care, including processing patient intake information;
  • To send the calls, texts, and emails you consented to receive about services, scheduling, and promotions;
  • To improve our website, content, and services using aggregate analytics;
  • To comply with legal obligations and protect the security of our website and patients.

We do not sell your personal information, and we do not share it for cross-context behavioral advertising.

4. Text and Email Communications

By providing your contact details and consenting on our consultation form, you agree that Forever Young LA may contact you by phone, text message, or email regarding services, scheduling, and promotions. Consent is never a condition of receiving care, and message and data rates may apply.

You can opt out at any time:

  • Text messages — reply STOP to any text message you receive.
  • Emails — click the unsubscribe link at the bottom of any marketing email.
  • Calls — tell any staff member during a call that you no longer wish to be contacted.

5. Sharing of Information

We do not sell or rent your personal information. We share information only in limited circumstances:

  • Service providers — companies that help us operate the website and communicate with you (such as website hosting, email delivery, and the Metricool analytics service). These providers receive only the information needed to perform their services and are contractually obligated to protect it.
  • Healthcare operations — disclosures of protected health information as described in our Notice of Privacy Practices, such as to business associates assisting us with treatment, payment, and health care operations.
  • Legal requirements — when required by law, subpoena, or government authority, or to protect the rights, safety, and security of our clinic, our patients, or the public.
  • Business transfers — in connection with a merger, sale, or transfer of all or part of our practice, subject to this policy.

6. Cookies and Tracking Technologies

Our website uses a tracking pixel provided by Metricool, our website analytics service, to measure page views and visitor engagement. The pixel collects aggregated, non-medical information such as the pages you visit, your browser type, and your general region. It does not collect your name, contact details, or health information.

You can control tracking by adjusting your browser settings to block third-party images or scripts, or by using browser privacy extensions. Our website does not currently respond to browser "Do Not Track" signals, but you can still block the tracking pixel using the methods above.

7. California Privacy Rights

If you are a California resident, California law provides you with specific rights regarding your personal information:

  • Know and access — you may request a copy of the personal information we hold about you.
  • Correct — you may ask us to correct inaccurate personal information.
  • Delete — you may ask us to delete the personal information we hold about you, subject to legal and medical-record retention requirements.
  • Non-discrimination — we will not deny services, charge different prices, or provide a different quality of service because you exercise any of these rights.
  • Opt-out of sale or sharing — we do not sell or share your personal information for cross-context behavioral advertising, so there is nothing to opt out of today. If that ever changes, we will update this policy and provide a "Do Not Sell or Share My Personal Information" link on our website.

You can also review and update the contact information you have provided by contacting us — see Section 12. We will respond to verifiable requests within the timeframe required by law.

8. Health Information (HIPAA)

Health information you provide through our patient intake questionnaire is protected health information subject to the Health Insurance Portability and Accountability Act (HIPAA). It is transmitted over an encrypted connection, stored with access limited to authorized clinic staff, and used only for treatment, payment, and health care operations as described in the Notice of Privacy Practices provided at our clinic. For questions about your health information, please contact our office — see Section 12.

9. Data Security and Retention

We use reasonable administrative, technical, and physical safeguards to protect your information, including encrypted transmission (HTTPS) and access controls on our systems. We retain personal information only as long as needed for the purposes described in this policy, or as required by applicable medical-records and other legal obligations, after which it is securely deleted or disposed of.

10. Children's Privacy

Our website and services are intended for adults. We do not knowingly collect personal information from children under 13. If you believe a child has provided us personal information, please contact us and we will delete it.

11. Links to Other Websites

Our website may link to third-party sites (such as our map, social media profiles, and our web design partner). We are not responsible for the privacy practices of those sites, and we encourage you to review their policies before providing them your information.

We may update this Privacy Policy from time to time. Material changes will be posted on this page along with a revised Effective Date. If we make changes that materially affect how we use information we already hold about you, we will notify you by email if we have your address on file.
